HUDSON FALLS – Thomas E. O’Leary, Jr., 63, of Hudson Falls, passed away on September 15, 2024.
Born on August 1, 1961, in Glens Falls, he was the son of the late Thomas O’Leary, Sr. and Nancy (McLaughlin) O’Leary.
Tom attended Lake George High School, and upon graduation, he attended and graduated from Westminster College in Utah.
In 1996, he married Wendy (Woodruff) Chakalis in Hudson Falls.
Tom proudly worked for over 31 years as a Registered Nurse at the Glens Falls Hospital.
He was predeceased by his parents.
He is survived by his wife, Wendy O’Leary; his daughter, Keara O’Leary; his son, Guy (Jennie) Chakalis; his grandson, Hunter Chakalis; his aunt Mary Warfel; his sister, Tammy Bottiger and her husband, Howard; and his nephew and several cousins.
Graveside services will be conducted 1 p.m. on Wednesday, September 18, 2024, St. Mary’s Cemetery in South Glens Falls.
The family requests that instead of flowers, donations be made to the charity of your choice.
My dear friend Tommy ‘ O, thank you for your friendship throughout the years. I cheerished every moment. One of the most nicest person I ever met. You will surely be missed.

I have the privilege of working with Tom at the hospital. I was a new nurse he was always so kind and he taught me so much. I am so sorry to he of his passing. I hope you find peace in your memories.
I had only known Tom since 2019, when I started working on T2, after the closure of 3West. Even though it was a short time, it was long enough for me to learn what a great guy & outstanding nurse he was! He discovered a few yrs. back that he knew my husband, as they had played billiards in the late 70’s @ Charlie’s Pool Hall on South St. Such great memories we will always treasure. This is such a trajectory & we’re so sorry for your loss Wendy.

So sad to hear he is gone too soon. I remember Tom as an excellent nurse who cared for his patients and coworkers with grace and poise.
